Go to the Reception desk where staff will greet you and confirm your details. We aim to ensure that your appointment happens on time. However, from time to time delays are unavoidable. Please be patient if this occurs.

What to Bring With You for your Consultation Visit

• General Practitioner referral letter – This is needed to be able to claim your rebate from Medicare for consultation appointments
• Health insurance details
• Medicare card
• Pension card
• DVA Card

Consultation – New Patients

If you are a new patient, you may like to complete the new patient detail form prior to your appointment. However there is also the option to complete the forms when you arrive.
